Well Visit Appointments
We see all patients on an appointment basis, and request that you call in advance so that we can reserve time for you.
The schedule of routine Well Child Exams is as follows:
| 3-7 Days Old | 12 Months of Age |
| 2 Weeks Old | 15 Months of Age |
| 2 Months of Age | 18 Months of Age |
| 4 Months of Age | 24 Months of Age |
| 6 Months of Age | 3 -18 Years – Yearly Physical |
| 9 Months of Age | New Patients to the practice for baseline |
State of Illinois Mandated Physical Exams Required for School:
- Most pre-school facilities - ages 3 & 4 years old
- Home day care settings – any age
- Kindergarten
- 6 th grade
- 9 th grade
Special Note: Yearly physicals are required at the junior high, middle school and high school levels for anyone participating in a competitive sport through the school.
The practice is accepting new patients.
